Trinity Behavioral Health can verify your qualifying Horizon Blue Cross Blue Shield PPO benefits for your own addiction treatment while separately reviewing your uninsured or non-covered partner for Trinity’s couples sponsorship model. Your Horizon benefits apply to you as the enrolled member. Trinity sponsorship is separate and does not make your partner covered by Horizon.

Whose treatment does Horizon cover?
Horizon benefits apply only to people who are actually enrolled and eligible under the plan. If only one partner is enrolled, Trinity verifies that person’s benefits, network status, cost sharing, and any authorization requirements.
How is my partner’s treatment handled?
If your partner is uninsured or not covered by your Horizon plan, Trinity may separately evaluate them for sponsorship. Sponsorship is a Trinity admissions arrangement and should not be described as Horizon paying for the uninsured partner.
