Trinity Behavioral Health can verify the insured partner’s qualifying Empire Blue Cross PPO benefits and may separately sponsor the other eligible partner through its couples sponsorship model. Empire is now branded Anthem Blue Cross and Blue Shield/Anthem Blue Cross in New York. The uninsured partner is not automatically covered by the insurer, and sponsorship must be reviewed separately.

What if only one of us has Empire PPO coverage?
Trinity verifies the enrolled member’s current Empire/Anthem plan for eligibility, network status, covered treatment, cost sharing, and authorization requirements. If the other partner is uninsured, Trinity may separately review that person for sponsorship.
