Select Kaiser PPO plans may provide addiction-treatment benefits for each covered partner who enters treatment as a couple, but each person’s eligibility and benefits must be verified separately. Trinity Behavioral Health accepts select Kaiser PPO plans only. Kaiser PPO plans are uncommon, and one partner’s policy or authorization does not automatically cover the other partner.

Does a Kaiser PPO include one shared couples-rehab benefit?
Do not assume so. Benefits are tied to the individual covered person and the exact KPIC PPO policy. Each partner can have separate eligibility, deductibles, coinsurance, provider-tier rules, precertification, and medical-necessity determinations.
What should be verified for each partner?
- That the policy is an eligible select Kaiser PPO arrangement
- Active enrollment
- The clinically appropriate covered level of care
- Trinity’s participating or nonparticipating status
- Precertification or prior authorization
- Medical-necessity criteria
- Deductible, copay, coinsurance, and out-of-pocket responsibility
