What does the COWS measure?
The scale considers signs and symptoms such as resting pulse, sweating, restlessness, pupil size, bone or joint aches, runny nose or tearing, gastrointestinal symptoms, tremor, yawning, anxiety or irritability, and gooseflesh.
What does the score mean?
The original COWS publication describes total scores from 0 to 47. Commonly used categories are mild withdrawal at 5–12, moderate at 13–24, moderately severe at 25–36, and severe above 36. These categories support clinical interpretation but should not replace a full assessment.
Why is COWS used during opioid detox?
A structured score can help clinicians document whether opioid withdrawal is emerging, improving, or worsening. It can also be useful when timing certain medications because some treatments can precipitate withdrawal if started too early.
Does COWS diagnose opioid use disorder?
No. COWS measures current withdrawal severity. Diagnosing opioid use disorder requires a broader clinical assessment of substance-use patterns, consequences, control, tolerance, and other criteria.
Can the score determine when medication should start?
It can contribute to the decision, particularly when clinicians are considering buprenorphine, but medication timing depends on the opioid involved, time since last use, clinical symptoms, and the treatment protocol. A number alone should not be used for self-treatment.
How often is COWS repeated?
